Self-Inspection Checklist for Packaging and Labeling of Toxic and Concerned Chemical Substances in Laboratories, National Chung Hsing University – Announcement (2025/05/13)
Please inform the responsible personnel of locations handling chemicals under your unit to carry out a self-inspection in accordance with the following instructions.

According to the Ministry of Education letters No. 1140051823 dated May 16, 2025, and No. 1140052606 dated May 19, 2025, and the Environmental Protection Administration announcements No. 1148108137 and No. 1148107690 dated May 13, 2025, please inform the responsible personnel of all chemical-handling locations under your unit to comply with regulations, conduct self-inspections, and update relevant records as required.

The Environmental Protection Administration (EPA) has newly designated the following substances as controlled toxic chemical substances:
(1) 5 new types of Perfluorooctanesulfonic acid (PFOS) and its salts and related compounds, and 352 new types of Perfluorooctanoic acid (PFOA) and its salts and related compounds are now classified as toxic chemical substances.
(2) 12 new nonylphenol substances and 28 nonylphenol ethoxylates have been added to the Chemical Abstracts Service (CAS) registry, and the regulatory concentration has been adjusted from 5% to 0.1%. -
The EPA has newly designated the following substance as a controlled concern chemical substance:
(1) N′-tert-Butyl-N-cyclopropyl-6-(methylthio)-1,3,5-triazine-2,4-diamine (Cyclobutrin). -
The containers and packaging for the aforementioned toxic and concern chemical substances must comply with the classification and labeling requirements set by CNS 15030 of the National Standards of the Republic of China. The labels must be clearly visible and include the following items with specified size requirements:
(1) Hazard Pictogram: A red-framed diamond on a white background, tilted at a 45-degree angle with a black symbol inside, sized for clear identification.
(2) Contents:
1. Name.
2. Hazardous components: Components exceeding regulatory concentration levels must be labeled with the name (in Chinese and English) and CAS number as announced by the central competent authority, along with the terms "Toxic Chemical Substance" or "Concern Chemical Substance" and the percentage by weight.
3. Warning phrases or precautionary statements.
4. Hazard warnings.
5. Precautionary measures.
6. Manufacturer, importer, or supplier’s name, address, and contact number.
(3) Label Size Requirements:
1. For containers ≤ 3 liters: not smaller than 52 mm × 74 mm (larger than A8 size).
2. For containers > 3 liters and ≤ 50 liters: not smaller than 74 mm × 105 mm (larger than A7 size).
3. For containers > 50 liters and ≤ 500 liters: not smaller than 105 mm × 148 mm (larger than A6 size).
4. For containers > 500 liters: not smaller than 148 mm × 210 mm (larger than A5 size).
5. For containers or packaging with special shapes, materials, or limited space, where standard labeling is difficult, foldable tags, hanging tags, or external packaging labels may be used instead. Label sizes are not restricted in such cases.
(4) In addition to the above requirements, labels must also include required warnings or supplementary information per regulations for toxic and concern chemical substance operations. -
Please confirm the operational status of the newly designated toxic and concern chemical substances. If there are remaining stocks, they must be properly registered and the internal layout must be promptly created or updated in the Chemical Management System.

Attached is the checklist for cross-checking operators holding newly designated toxic and concern chemical substances. Please complete the form, print it in color with updated status markings, and submit it to this center before 5:00 PM on Friday, June 13, 2025. Each site handling toxic and concern chemical substances must carry out the following actions:
(1) Ensure proper management of toxic and concern chemical substances, and record daily operations in compliance with regulations (Control Cards and the Chemical Management System). Violations may be fined NT$100,000 to NT$500,000 in accordance with Article 58, Paragraph 1 of the Toxic and Concerned Chemical Substances Control Act.
(2) Containers, packaging, operation sites, and facilities must display the required toxic and pollution prevention labels, and have Safety Data Sheets (SDS) available for all toxic and concern chemical substances. Violations may be fined NT$60,000 to NT$300,000 under Article 59, Paragraph 9.
(3) In the event of an incident, the site supervisor must take immediate emergency control measures and report to the local Environmental Protection Bureau within 30 minutes:

The responsible person must also clean up according to regulations and submit a written incident investigation report. Failure to comply may result in fines of NT$1 million to NT$5 million, per Article 55, Paragraph 6.
(4) Given the severity of penalties and the potential damage to the university’s reputation, any fines resulting from violations will be borne by the responsible unit. All other unlisted matters should be handled in accordance with relevant laws and regulations regarding the management of toxic and concern chemical substances.
